🚨 MYTH: Tape alone is enough to secure an airway device.

✅ FACT: Whilst tape does secure an airway device, it is an infection risk to the patient. In the eyes of the National Infection Prevention Control Manual multi-patient rolls of tape are by definition, classified as non-invasive re-usable equipment, meaning they should be decontaminated to adhere to National Infection Control Standards. This is NOT happening. Tapes are in fact not fit for purpose and if risk assessed would not meet the demanding regulation required to get a medical device to the market. Healthcare governance must establish a consistent policy regarding the use of products that are not designed for their intended purpose. The repeated claim that the NHS embraces innovation is clearly unfounded when it comes to airway securement.

🔑Leafix Key training points

✔ When applying Leafix, make sure the skin is dry. The product will cope with sweat and fluids as long as it is put on dry skin.

✔ Place the Y shape anchor point first and close to the side of the mouth, so you do not have to stretch the material.

✔ Make sure the Leafix does not have creases or gaps particularly if the patient is Prone, this will prevent fluid or saliva getting under and affecting the adhesive.

By following these steps, you will ensure that the Leafix is used correctly and to it's full potential!

📚 This week's #trainingtuesday looks at the sel 📚 This week's #trainingtuesday looks at the selective surgeries proving popular with Leafix.

Our goal with Leafix is eradicate the use of tape and ties in theatres completely. However, in these early stages we have seen a higher uptake in selective surgery types where problems with off-label conventional materials such as tape and ties are more prevalent.

🧠Neuro - When the patient is prone for long periods, ties can cut into the corners of the mouth and cause severe facial damage. Leafix as an adhesive product doesn't pose this risk but will provide a much safer alternative to tapes.

🫀Cardiac procedures are lengthy and when the patient is transferred to ICU - Leafix can streamline the airway fixation method, providing security and consistency through the entire period of the intubation.

👂🏻👃🏻ENT when access is required to parts of the facial anatomy and movement of the tube can compromise security. The flexible design of Leafix ensures it can be adapted to suit any circumstances.

🦾Any Robotic procedure done in the steep Trendelenburg position. Ties are not advised to secure the airway in this position as they wrap around the back of the head and can contribute to neurological impairment following the surgery. Leafix is proving to be a safer securement method in this case.

📚 This week's #trainingtuesday looks at the com 📚 This week's #trainingtuesday looks at the commonly used standard endotracheal tube.

💡 The design of Leafix allows for a flexible approach when it comes to securing the ETT and often the best method of securement is dictated by specific circumstances, such as surgery type, patient type, facial hair etc.
 
📝 We often advise the over-and-under technique when securing the airway as it facilitates securement to the maxilla (upper jaw) on the opposite side of the face. As there is less movement compared to the mandible (lower jaw) this in theory offers a better securement over longer periods.

A microlaryngeal endotracheal tube (MLT) size 5 is used to improve visibility and access to the surgical site during microlaryngeal or tracheal surgery. The tube has a smaller diameter than a standard ETT and is longer in length so that it can be used for patients with narrowed airways.

In our ongoing efforts to change clinical practices and ensure patient safety, it is vital to evaluate the materials and methods we use in everyday procedures. One area that warrants attention is the use of cotton ties for securing airways. Despite their common use, these ties present several challenges that can impact patient care, procedural efficiency and even life!

As we highlighted in our post regarding tape – the issue is essentially the same here. Ties are not designed to secure an airway, nor would they be accepted under the new medical device regulations as a safe way of performing airway securement (which makes a mockery of the whole system seeing as ties are used every day!)

❗ Ties need to be wrapped around the back of the head. Any securement solution that needs to be wrapped around the back of the head can cause occlusion of venous drainage – potentially contributing to cognitive dysfunction in cases where the patient is in the steep Trendelenburg position.
 
❗ Ties cause various pressure points, not least at the corners of the mouth and in longer procedures this can and has led to severe facial injuries.
 
❗ Cut to size before use: Precision in cutting the ties to the correct length is crucial, yet this process can be time-consuming and imprecise.
 
❗ Various knots used: Different knotting techniques can result in inconsistent tension, which may affect the security of the airway.
 
❗ Moisture absorbent: Cotton’s moisture-absorbing properties can compromise its strength and hygiene over time.

To the unsuspecting eye, this is a harmless roll o To the unsuspecting eye, this is a harmless roll of tape. A useful tool, and a capable ally in various circumstances. However, every day in Operating Theatres all around the world this is used to secure one of the most vital pieces of equipment in any general anaesthetic - the patient airway. 
 
😮 Yes, you read that right; healthcare practitioners all around the world use multi-purpose rolls of tape to ensure that the patient's lifeline is anchored to their face to reduce the chance of it dislodging during a procedure - a potentially catastrophic event that can lead to mortality and complications.
 
If you're not shocked by that then you should be, but let's take a closer look:
 
Quite simply, tape is not designed to secure an airway. It is designed to break easily and healthcare staff are forced to develop their own method to secure an airway with no guidance from any recognised bodies.

🤔 Ask yourself this - would a roll of tape make it through the new medical device regulations and be passed as safe to perform such a vital function in airway securement?

❗ Tape is supplied by a variety of manufacturers with varying adhesive strengths meaning you can never be assured that the quality of the tape will hold the airway in place. 

❗ It is harmful the to skin and can often lead to redness and damage, particularly in patients with sensitive or frail skin.

❗ Tape is an infection risk. It is carried in pockets, stuck to theatre scrubs, kicked around the coffee room.... and then stuck to the patient's face in what is supposed to be a clean environment where the highest level of healthcare is provided
 
With all of the above said, there has never been an alternative in Operating Theatres and this bad practice has been overlooked for generations, ingrained in culture. The time for change is now.

When the patient is prone, airway security is of utmost importance and there are several additional concerns. However, the purpose of Leafix is to secure all airways in all surgical positions and therefore can handle this challenging circumstance.

📝 We advise that two Leafix are used to perform the bilateral trouser-leg technique. This provides the highest level of security and safety!

✔ Peel the Leafix from the backing card and place the Y shaped anchor section on the patients cheek, near the side of the patient's mouth

✔ Wrap under and around the endo- tracheal tube, following which, split the Leafix along its perforation and secure the lower limb to patient's face

✔ The other limb can now be wrapped around and up the airway device to offer extra securement

✔ Repeat the process in the same way with a second Leafix device from the opposite side of the face

🔒 The airway device is now secure for the prone position
